Irradiating Apparatus Licence and Application Procedures

• You may use form IA-P-R2 to apply for the renewal of a licence that permits you to possess an irradiating apparatus in a functional state.

Guidance notes for IA-P-R2 form

General

• Submit this application at least 8 weeks before licence expiry.

Part A

1. Fill in the current irradiating apparatus licence number at top of the form.
2. Tick the appropriate box. If all relevant details remain unchanged, go to part E.
3. Complete either A1 or A2 and rest of the questions in Part A :

   For Section A1, please submit

   1. 2 recent photos of size 45 x 60 (mm).

   For Section A2, please submit

   1. A copy of the business registration certificate.
   2. A copy of Identity Card / Document of the authorized person (if changed).

Part B

Fill in detailed information of the irradiating apparatus under possession.

Part C

Tick the appropriate boxes and fill in new information if necessary.

Part D

Specify at least one person who is qualified, and who is holding a valid licence, to use the concerned type of irradiating apparatus.

Part E

Sign the declaration.

• You may use form IA-P-R1 to apply for the renewal of a licence that permits you to store irradiating apparatus or X-ray Tube at a specified premises.

4. Complete either A1 or A2 and rest of the questions in Part A :

• Use IA-U2 form to renew licence to use an irradiating apparatus for purposes not affecting human body (e.g. veterinary X-ray machines, industrial radiography, portable XRF, etc).

• Submit this application at least 8 weeks before licence expiry.

3. If you are an employee, fill in detailed information of your employer.

• Use IA-S form to renew licence that permits the sale of irradiating apparatus.

General

• Submit this application at least 8 weeks before licence expiry.

Part A

1. Fill in the current irradiating apparatus licence number at the top of the form.
2. Complete all items in Part A :

   Submit

   1. A copy of the business registration certificate.
   2. A copy of Identity Card / Document of the authorized person.

Part B

B1 :

1. Tick the appropriate box as necessary.
2. Submit sales return including the names, address and irradiating apparatus licence numbers of the purchasers, the irradiating apparatus(es) sold and dates of sales.

B2 :

Tick the appropriate boxes that suit your business activity. You may take more than one box.

Part C

1. Fill in relevant information for the irradiating apparatus to be sold (type of irradiating apparatus).
2. Submit a duly completed irradiating apparatus assessment form together with technical specifications of the irradiating apparatus, in particular those relating to radiological protection and test reports showing conformance with international standards (this submission is mandatory for any irradiating apparatus to be imported and sold to users in Hong Kong).

Part D

Tick the appropriate box for renewal of licence and fill in the licence number.
