Application for New Licence
Licence for Possession of an Irradiating Apparatus in Non-functional State
- This licence is required to cover the possession of irradiating apparatus at specified premises where it is being installed.
- Use of the irradiating apparatus is not allowed under this licence.
- This type of licence is valid for 12 months or until replaced by a licence for possession of irradiating apparatus in functional state, whichever first occurs.
Guidance notes for IA-P1 form
General
- You need one possession licence for each irradiating apparatus.
- Submit separate application for each irradiating apparatus.
Part A
Complete either A1 or A2 and rest of the items in Part A :
For Section A1, please submit
- A copy of Identity Card / Document of the applicant.
- 2 recent photos of size 45 x 60 (mm).
For Section A2, please submit
- A copy of the business registration certificate.
- A copy of Identity Card / Document of the authorized person.
Part B
Tick the appropriate boxes relevant to your application :
- Fill in the seller's information and irradiating apparatus licence number if the irradiating apparatus is purchased from a local company. The company shall have valid irradiating apparatus licence to sell the irradiating apparatus as referred in this application.
- If the irradiating apparatus is imported directly from other countries,
- please submit a duly completed irradiating apparatus assessment form together with technical specification of the irradiating apparatus.
- You also need to apply import licence for the irradiating apparatus.
Part C
Fill in relevant information for the irradiating apparatus (Type of irradiating apparatus).
Part D
- Tick the appropriate box and fill in the address for installation of the irradiating apparatus.
- Fill in the information of the premises.
Part E
- You need to complete this part only if the irradiating apparatus is to REPLACE an existing irradiating apparatus at the premises.
- Fill in item 1 and 2. State clearly how to deal with the existing irradiating apparatus.
Part F
Tick the appropriate box and sign the declaration.

Licence for Storage of an Irradiating Apparatus or X-ray Tube
- This licence only permits the storage of irradiating apparatus or X-ray Tube at a specified premises.
- Use of irradiating apparatus is not allowed
Guidance notes for IA-P1 form
General
- You need one possession licence for storage of each irradiating apparatus.
- Submit separate application for each irradiating apparatus.
- Only one licence is needed for storage of x-ray tubes.
Part A
Complete either A1 or A2 and rest of the items in Part A :
For Section A1, please submit
- A copy of Identity Card / Document of the applicant.
- 2 recent photos of size 45 x 60 (mm).
For Section A2, please submit
- A copy of the business registration certificate.
- A copy of Identity Card / Document of the authorized person.
Part B

- You need a licence for possession of each irradiating apparatus in functional state
- Submit separate application for each irradiating apparatus.
Guidance notes for IA-P2 form
General
- You need licence for each irradiating apparatus.
- Submit separate application for possession of each irradiating apparatus in functional state.
- Submit with this application form a floor plan of the premises showing :
- the location of the irradiating apparatus within the room,
- the dimensions of the room,
- the type and thickness of the construction material of the door(s), walls, floor and ceiling of the room,
- direction of the primary radiation beams, and
- utilization of the adjacent areas, e.g. office, reception, corridor, darkroom, store room, etc. (Sample of floor plan drawings)
- Submit documentary evidence to show the ownership of the irradiating apparatus.
Part A
Complete either A1 or A2 and rest of the questions in Part A :

- A copy of Identity Card / Document of the applicant.
- 2 recent photos of size 45 x 60 (mm).
For Section A2, please submit
- A copy of the business registration certificate.

- Fill in detailed information of the irradiating apparatus in the table.
- Tick the appropriate boxes relevant to your application.
- Submit technical specifications of the irradiating apparatus with your application.
Part C
- Tick the appropriate box related to shielding of the irradiating apparatus.
- Fill in shielding information for floor, ceiling, walls and doors as required.
Part D
Tick the boxes on personal protective measures available in the premises. You may tick more than one box.
Part E
- Specify at least one person who is qualified, and is holding a valid licence, to use the concerned type of irradiating apparatus.
- Tick the appropriate box if contrast media is intended to be used.
Part F
Sign the declaration.

Licence for Use of Irradiating Apparatus on Human Body
- This licence permits the use of irradiating apparatus on human body.
Guidance notes for IA-U1 form
General
Submit with this application form :
- A copy of Identity Card / Document of the applicant.
- 2 recent photos of size 45 x 60(mm).
- A copy of current professional practising certificate of every stated qualification.
Part A
- Fill in relevant information and your qualification in radiation safety.
- If you are an employee, fill in detailed information of your employer.
Part B
Tick the appropriate box for new application of licence.
Part C
Sign the declaration.

Licence for Use of Irradiating Apparatus NOT on Human Body
- Use of the irradiating apparatus on human body is not allowed for this type of licence (e.g. veterinary X-ray machines, industrial radiography, portable XRF, etc).

General
Submit with this application form :
- A copy of Identity Card / Document of the applicant.
- 2 recent photos of size 45 x 60(mm).
- A copy of certificate(s) of every stated qualification(s).
Part A
- Fill in relevant information and your qualification in radiation safety.
- If you are an employee, fill in detailed information of your employer.
Part B

Licence for Sale of Irradiating Apparatus
- This licence permits the sale of irradiating apparatus.
Guidance notes for IA-S form
General
Submit a duly completed irradiating apparatus assessment form together with technical specifications of the irradiating apparatus, in particular those relating to radiological protection and test reports showing conformance with international standards (this submission is mandatory for any irradiating apparatus to be imported and sold to users in Hong Kong).
Part A
Complete all items in Part A :
Submit
- A copy of the business registration certificate.
- A copy of Identity Card / Document of the authorized person.
Part B
- B1 is for licence renewal only.
- B2: Tick the appropriate boxes that suit your business activity. You may tick more than one box.
Part C
